Navigating Your Well-Being
Older adults face distinct health and wellness challenges. Our health and wellness programs are designed to support both you and your caregiver in improving your quality of life. One of the primary ways our Health Promotion and Disease Prevention (HPDP) team engages with our network is through evidence-based workshops tailored to meet these needs.
Evidence-Based Workshops Defined
Evidence-based programs are research-based courses that provide documented health benefits, so you can be confident they’ll work for you or your loved one. We offer five current evidence-based workshops.
Living Healthy with Chronic Disease Self-Management Program
Living Healthy empowers people with chronic conditions such as diabetes, arthritis and high blood pressure to live fulfilling lives. This program is designed to boost participants’ confidence to actively self-manage their health condition.
Techniques included in this workshop address fatigue, frustration, medication, communication with healthcare providers, decision making and more.
Living Healthy with Diabetes Self-Management Program
For people hoping to prevent or live with diabetes and their caregivers, the DSMP workshop provides techniques to self-manage symptoms associated with diabetes. Mutual support and group interactions boost participant confidence in using self-management techniques.
Techniques included in this workshop address depression, hyper and hypoglycemia, healthy eating, exercise and more.
Living Healthy with Chronic Pain Self-Management Program
Chronic pain shouldn’t control your life. Learn how to manage a wide range of pain conditions and confidently maintain an active life.
Techniques included in this workshop address poor sleep, fatigue, balancing activity and rest, medication and more.
Building Better Caregivers
Those who care for an older loved one need support. These workshops are designed for caregivers who want to reduce stress, manage challenging behaviors and connect with valuable resources. Open to all caregivers, participants learn self-management tools to address issues and events associated with caregiving. Mutual support and shared experiences strengthen caregiver confidence while participants create action plans that set them and their loved ones on a path to success.
A Matter of Balance: Managing Concerns About Falls
It’s common for older adults to live in fear of falling, but it doesn’t have to be this way. Fear of falling can result in limited activity, physical weakness, isolation, and an increased risk of falls. Build confidence in your own mobility by learning how to navigate your environment better and how to fall in a way that reduces the risk of serious injury. A Matter of Balance emphasizes practical strategies that set realistic goals, improve assertiveness and make falls more manageable.
Each workshop contains six to eight sessions, with one session held each week. In-person and virtual options are available.
